Consent for Treatment

As part of our philosophy of care we acknowledge your right as a patient to make decisions about your treatment. Before you give your consent for treatment, your doctor will explain the nature of the treatment and its potential side effects. Take this time to ask questions if you are uncertain about what is going to happen to you.

You will be asked to sign a privacy consent form on admission. Details regarding privacy in health care are available from information and Privacy Commission NSW on their website or by phoning them on (02) 8019 1600

Your feedback is important to us. Please take some time to complete a Patient Satisfaction Questionnaire.  Contact us for a copy. If you have any areas of concern, please do not hesitate to speak with the staff or our Consumer Representatives.
